What You'll Do As a Home Health Physical Therapist, You Will Evaluate patients in their homes and assess physical abilities and functional limitations.Develop individualized treatment plans based on each patient's goals and needs.Provide skilled physical therapy to restore, maintain, or improve physical function.Help patients manage pain and prevent further disability or functional decline.Educate patients and families on exercises, mobility, safety, and strategies for continued progress.Collaborate with interdisciplinary care teams to provide a coordinated patient experience.Monitor patient progress and adjust treatment plans as appropriate.Help patients achieve greater independence and quality of life.
