About the Role
As a Physical Therapist in Home Health, you will assess patients' functional status, mobility, pain, and medical conditions to develop and implement personalized treatment plans. You will provide therapeutic interventions, monitor patient progress, and collaborate with physicians and the healthcare team to optimize patient outcomes.
Responsibilities
- Evaluate patients' needs and assist in creating therapy plans of care
- Deliver services within state practice laws and plan of care
- Observe and report patient responses and condition changes
- Treat patients to relieve pain, restore function, and maintain maximum capacity
- Educate patients, families, and healthcare team members about care plans and progress
- Visit patients, document progress, and complete evaluations per guidelines
- Report evaluation outcomes, goals, and care frequency
- Adapt to diverse home environments and patient backgrounds
Requirements
- Current Physical Therapy license in Washington
- Valid driver's license, vehicle insurance, or access to transportation
Preferred Qualifications
- CPR certification or ability to obtain within 90 days
