Physical Therapist - Home Health

Job Details

Pride Health is hiring a Physical Therapist to support their client's home health program in Batesville, AR (Coverage Areas: 50-mile radius surrounding Batesville, AR).

This is a 13-week assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!

Job Responsibilities

• Perform home health PT evaluations, reassessments, and Start of Care (SOC) visits.
• Develop and implement individualized treatment plans based on patient needs and physician orders.
• Provide skilled physical therapy treatment visits when required.
• Complete accurate and timely clinical documentation in HCHB.
• Educate patients and caregivers on treatment plans, safety, mobility, and home exercise programs.
• Assess patient progress and adjust treatment interventions accordingly.
• Collaborate with interdisciplinary care teams to ensure continuity of care.
• Maintain compliance with agency policies, documentation standards, and home health regulations.

Qualifications

• Current Physical Therapy license in the state of practice.
• Home Health experience required.
• Experience with Homecare Homebase (HCHB) required.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.

Pay and Schedule

• Pay rate: $50-56/hr. (On W2, Pay Rate is based on experience).
• Schedule: Mon - Fri (5 days/wk.) 8 AM to 4:30 PM (32-40 hrs. weekly guaranteed).

Mileage

• Mileage: Reimbursement provided (from first patient to last patient).

On Call/Weekend

• No On Call/Weekend.
