About the role

The successful candidate will provide physical therapy services to patients in their homes, focusing on improving mobility and managing chronic conditions.

Responsibilities

• Assess patient needs and develop individualized treatment plans
• Provide hands-on physical therapy treatments
• Document progress and communicate with healthcare providers
• Maintain a safe and clean work environment

Requirements

• Bachelor’s degree in Physical Therapy from an accredited program
• Current license to practice as a Physical Therapist in the state
• At least 1 year of relevant clinical experience

Qualifications

• Strong interpersonal skills
• Excellent organizational and time management abilities
• Ability to travel to various locations within the service area

Skills

• Proficient in using electronic health records (EHR)
• Knowledgeable in treating common orthopedic and neurological conditions
