PHYSICAL THERAPIST - HOME HEALTH

Key Responsibilities

• Conducts physical therapy assessments and skilled treatments
• Coincides client care coordination
• Teaches, supervises, and counsels clients and caregivers
• Maintains documentation, communicates interdisciplinary updates, supervises ancillary staff, and ensures safe patient transfers
• Develops and manages plans of care with effective time management and critical thinking skills

Qualifications & Skills

• Must assist with patient lifts, transfers, and functional activity training
• Minimum requirement is a Bachelor's degree; a Master's or Doctorate is preferred
• Essential skills include clear communication, confidentiality, crisis management, customer service, electronic health records proficiency, and a valid driver's license with minimal violations
• Credentials include a PT license and BLS certification
