Physical Therapist Home Health
About the role
The Physical Therapist at CHI Health Connect Home-Fargo will provide compassionate, personalized care in the home. They will evaluate and treat disabilities, injuries, and disease using a range of manual therapy, modalities, and exercises. The Physical Therapist will also provide education and consulting services, comply with regulatory standards, and supervise Physical Therapist Assistants and support personnel.
Responsibilities
- Evaluate and treat patients referred by providers
- Provide appropriate supervision for Physical Therapist Assistants and support personnel
- Complete documentation related to patient care in a timely manner
- Participate in and provide educational in-services for staff and providers
- Collaborate with healthcare team and provide professional recommendations regarding patient care
- Participate in patient related care coordination activities (care conferences, etc.)
Requirements
- Possesses a degree from a baccalaureate, Masters or Doctorate program in physical therapy approved by an accredited organization
- Licensed to practice as physical therapist within the State of ND
- Two years of appropriate experience as a physical therapist
- Possesses and maintains current CPR certification
- Current MN Driver’s license
Preferred
- Community/home health experience

Pay
The posted compensation range of $23.05 - $32.56 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting.
Schedule
Monday-Friday, 80 hours per week, Day shift, No remote work.
