Physical Therapist - Home Care (PRN)

About the Role

We are seeking a dedicated Physical Therapist (PT) to join our Home Health team in a PRN capacity, serving patients across the Greater Nashville Area. This role provides autonomy, weekday flexibility, and the support of a nationally recognized health system. Whether you’re experienced in home health or exploring a new specialty, this role offers the opportunity to help patients regain independence where it matters most—their home.

Service Area: Greater Nashville Area, northern focused.

Responsibilities

• Provide high-quality, evidence-based physical therapy in patients’ homes.
• Deliver care according to individualized treatment plans developed by supervising PTs.
• Promote patient safety, mobility, and functional independence.
• Educate patients and caregivers on exercises, mobility strategies, and home safety.
• Accurately document clinical notes and patient progress in EMR systems.
• Communicate effectively with interdisciplinary team members.
• Travel independently to patient homes within designated service areas.
• Analyze comprehensive information from formal and informal assessments to determine a rehabilitation treatment diagnosis.
• Collaborate with stakeholders in establishing the plan of care, coordinate and implement care delivery while anticipating patient care needs across the rehabilitation continuum.
• Apply appropriate, evidence-based interventions to ensure effective outcomes and re-evaluate performance to assess patient's progress towards goals.
• Employ strategies to promote successful return to home, community, and school/vocational environments.
• May supervise and direct certified athletic trainers (PT only), physical therapist/occupational therapist assistants, technicians, volunteers, and students.

Requirements

• Active Physical Therapist (PT) license in the state of Tennessee.
• Minimum 1 year of PT experience.
• Strong communication and assessment skills.
• Ability to work independently with a flexible PRN caseload.
• A valid driver’s license, reliable transportation, and ability to maintain a satisfactory driving record.

Preferred Qualifications

• Home Health experience.
• Experience with EMR documentation systems.
• Strong understanding of rehabilitation principles and functional mobility.

Skills

• Rehab Therapist Patient Documentation (Novice): Demonstrates proficiency in documentation practices. Documents in the appropriate templates and utilizes only approved abbreviations within patient documentation. Meets institutional standards for quality and timeliness of documentation.
• Treatment Planning & Goal Setting (Novice): Demonstrates sufficient proficiency in developing a plan of care that incorporates treatment interventions focused on meeting objective goals. Develops and documents patient-focused short and long-term goals that are functional and measurable. Identifies the need for and referral to other services, as needed.
• Rehabilitation Therapist Patient Evaluation (Novice): Demonstrates proficiency in evaluating patients with physical deficits including impairments, developmental disabilities, functional limitations, disabilities, or changes in physical function by obtaining pertinent information and administering standardized tests and objective measures. Selects appropriate measures and performs a functional assessment to document patient's status requiring skilled therapy. Interprets assessment results and their significance to the patient, family, referring physician, and healthcare team.
• Treatment Implementation (Novice): Demonstrates proficiency in providing treatment consistent with physician prescription and established plan of care. Modifies treatment plans as needed to demonstrate provision of skilled therapy services, including progression of treatment and revision of interventions if goals are not met. Selects and follows correct protocols for care and utilizes evidence-based treatments.
• Physical/Occupational Therapist Patient Education (Novice): Demonstrates proficiency in communicating functional impact of assessment findings, treatment interventions, treatment planning diagnosis, and disease process. Considers communication needs and level of understanding of the audience and may share appropriate information/literature. May supervise, educate, and train students from professional programs of physical therapy, occupational therapy, and athletic training.

Training & Orientation Schedule Expectations

• New Hire Training: Held on Mondays.
• Clinical Orientation: Conducted 3 days per week during the onboarding period (6-8 weeks, depending on experience).
