Schedule

Full-Time, Monday–Friday, 8:00 AM–4:30 PM (no weekends or holidays).

Pay

Targeted pay range: $50.00 – $60.00 per hour, depending on years of experience. Ecumen considers a variety of factors when determining new hire pay, including experience, qualifications, certifications, licenses, and education.

Responsibilities

• Provide comprehensive assessments and treatment planning to meet patient goals and deliver quality care by evaluating and interpreting test results, determining treatment plans in consultation with physicians, and following prescriptions.
• Help patients accomplish treatment plans; secure necessary supplies and adaptive equipment to facilitate independence.
• Administer therapy statements according to a treatment plan approved by the attending physician; direct treatments given by aides and assistants.
• Evaluate effects of therapy treatments through observation, documentation, and progress assessment; recommend and implement adjustments and modifications.
• Complete discharge planning by consulting with physicians, nurses, social workers, and other healthcare workers; contribute to patient care conferences.
• Ensure continuation of therapeutic plans by designing and instructing patients, families, and caregivers in home exercise programs; recommend and/or provide assistive equipment.
• Document patient care services accurately in patient records; maintain detailed records of services rendered for billing purposes.
• Supervise and develop physical therapy staff; maintain professional and technical knowledge.
• Follow guidelines and requirements set by CMS, federal and state regulations, and other payors (insurance companies).
• Manage and maintain individual license/certification renewals and other professional certifications as applicable.
• Perform other duties as assigned.

Requirements

• Bachelor’s degree in Physical Therapy
• Previous experience working as a Physical Therapist
• Current license, state certification, or registration as a Physical Therapist in good standing
• Valid driver’s license, proof of insurance, and satisfactory motor vehicle record
• Ability to communicate effectively in both verbal and written formats
• Flexibility and adaptability to changing situations, including working irregular hours, attending meetings, and special events
• Ability to follow all safety rules, regulations, policies, and procedures, including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident’s Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to Know, SDS, ADA, HUD Policy and Procedures, and Fair Housing laws and regulations

Preferred Qualifications

• Doctor of Physical Therapy (DPT) degree
• 2+ years of previous experience in home health, hospice, or senior care

Driving Requirements

Local travel required; valid driver’s license and proof of insurance necessary.
