As a Home Care Physical Therapist, you will provide quality physical therapy services through comprehensive evaluation, program planning, and administration of treatment. You will perform detailed clinical assessments based on individual patient needs upon physician referral and develop a comprehensive plan-of-care following physical therapy evaluation, helping patients regain physical mobility in the comfort of their own home.

Responsibilities

• Perform detailed clinical assessments based on individual patient needs upon physician referral.
• Develop a comprehensive plan-of-care based on findings from physical therapy evaluations.
• Administer physical therapy treatments to patients in their homes.

Requirements

• 1 year of clinical experience or Clinical Affiliation in Home Care.
• Graduate of a Physical Therapy program (Bachelor’s Degree required; Master’s or Doctorate preferred).
• Current Pennsylvania Physical Therapy license.
• Provider BLS (American Heart Association/American Red Cross) certification required on hire.
• Valid Driver’s license and insured automobile.

Schedule
