About the role

FOX Rehabilitation is growing in Edina, MN and seeks licensed Physical Therapists passionate about delivering exceptional one‑on‑one care to older adults where they live. You will treat patients in private residences, senior living communities, independent living, assisted living, or memory care settings using the Geriatric House Calls™ model. This role offers autonomy, flexibility, and the opportunity to create and manage your own treatment schedule while building strong patient relationships. Telehealth and virtual care options are available to enhance flexibility and continuity of care.

Responsibilities

• Provide one‑on‑one physical therapy to older adults in private residences, senior living communities, independent living, assisted living, and memory care settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence‑based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
