Physical Therapist - Full-Time

About the role

Licensed Physical Therapist responsible for delivering personalized, in-home and senior living care to older adults, supporting mobility, independence, and quality of life.

Responsibilities

• Provide one-on-one physical therapy sessions in private homes and senior communities.
• Perform evaluations, develop individualized treatment plans, and deliver evidence-based interventions.
• Support patient wellness, functional independence, and safety through therapy and education.
• Document patient progress efficiently within EMR systems.
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
• Incorporate telehealth and virtual care to enhance access and continuity of care.

Qualifications & Skills

• Valid Physical Therapy license or eligibility.
• Degree from an accredited program.
• Proficiency with computer systems and EMR.
• Current CPR certification.

Physical Requirements

• Ability to exert moderate effort.
• Assist with patient transfers.
• Perform CPR.
• Drive between patient locations, with appropriate PPE use.
