Physical Therapist - Full-Time

Role: Licensed Physical Therapist responsible for delivering personalized, in-home and senior living care to older adults, supporting mobility, independence, and quality of life. Key Responsibilities Provide one-on-one physical therapy sessions in private homes and senior communities.Perform evaluations, develop individualized treatment plans, and deliver evidence-based interventions.Support patient wellness, functional independence, and safety through therapy and education.Document patient progress efficiently within EMR systems.Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.Incorporate telehealth and virtual care to enhance access and continuity of care. Qualifications & Skills Valid Physical Therapy license or eligibility.Degree from an accredited program.Proficiency with computer systems and EMR.Current CPR certification. Physical Requirements: Ability to exert moderate effort, assist with patient transfers, perform CPR, and drive between patient locations, with appropriate PPE use.
