Physical Therapist - Full-Time

Role: Licensed Physical Therapist providing in-home and senior living care, with optional telehealth components, to older adults. Responsibilities: Conduct evaluations, develop personalized treatment plans, and deliver evidence-based therapy sessions to improve mobility, independence, and quality of life. Document progress accurately in EMR, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff on safety and aging-in-place strategies. Support virtual care when appropriate. Qualifications & Skills: Valid PT license or eligibility, degree from an accredited program, proficiency with EMR and basic computer skills, and current CPR certification. Must meet physical demands including lifting, repositioning patients, and driving to patient locations. Key Duties: Provide one-on-one therapy in various settings, develop individualized care plans, and support patient wellness through therapeutic activities. Foster strong patient relationships and contribute to community-based, holistic care.
