About the role
We are seeking full-time Physical Therapists to join a team focused on providing Geriatric House Calls™ in community settings. The role offers the autonomy to create and modify treatment plans within a collaborative interdisciplinary team, delivering 1:1 physical therapy services aimed at achieving functional outcomes and promoting aging in place.
Responsibilities
- Deliver 1:1 physical therapy services to geriatric patients in community settings
- Develop and modify treatment plans to achieve functional outcomes
- Promote aging in place through personalized care
- Collaborate with an interdisciplinary team
Requirements
- Valid Physical Therapy license in the state of practice
- Degree from an accredited Physical Therapy program
- Basic computer skills
- Current CPR certification
