Responsibilities

• Deliver one-on-one evaluations, develop care plans, and implement evidence-based treatments to enhance mobility, independence, and quality of life.
• Utilize telehealth and virtual care options for greater flexibility and continuity.
• Collaborate with interdisciplinary teams, educate patients, caregivers, and staff to promote safety and aging in place.
• Manage your own schedule with autonomy and flexibility, supporting meaningful patient relationships and outcomes.

Qualifications

• Valid state licensure
• Degree from an accredited program
• Computer proficiency
• CPR certification
