About the role
Provides physical therapy services to restore and improve patients' physical, social, and mental functions within a home health and hospice setting.
Responsibilities
- Conduct evaluations to assess patient needs and develop individualized treatment plans.
- Deliver therapy interventions using appropriate protocols and techniques.
- Create care and discharge plans, ensuring clear communication with relevant parties.
- Complete accurate clinical documentation within specified deadlines.
- Supervise assigned assistants and perform other related duties as needed.
Requirements
- Doctor of Physical Therapy (DPT) degree or foreign equivalent.
- Valid physical therapy licensure in applicable states.
- CPR or BLS certification from an approved provider.
- Valid driver’s license and age of at least 21 years.
