About the Role

We are seeking licensed Physical Therapists to provide personalized, one-on-one care to older adults in private homes and senior living communities across the Greater Boston area. The role involves conducting evaluations, developing individualized treatment plans, and delivering evidence-based therapy to improve mobility, independence, and quality of life, primarily through our Geriatric House Calls™ model.

Responsibilities

  • Perform assessments, create treatment plans, and deliver therapy sessions supporting patient wellness and functional independence.
  • Document care accurately within the EMR system, ensuring regulatory compliance.
  • Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
  • Educate patients, caregivers, and staff to promote safety and aging in place.
  • Utilize telehealth and virtual care to extend access and flexibility.

Requirements

  • Valid Physical Therapy license or eligibility in practice state(s).
  • Degree from an accredited Physical Therapy program.
  • Proficiency in basic computer skills and EMR systems.
  • Current CPR certification.

Physical Requirements

Ability to exert medium physical effort, assist with patient transfers, reposition patients, and perform therapeutic activities, including driving to patient locations and wearing PPE.
