About the role

We’re looking for a Physical Therapist to take on a full-time role with an established home health provider in Seattle, WA. You will help patients move better, feel better, and stay active—whether they are recovering from an injury or managing long-term conditions.

Responsibilities

  • Provide physician orders and implement plan of care under the direction and supervision of the Director of Clinical Services.
  • Work with other home care personnel in interdisciplinary patient care planning.
  • Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
  • Take initial patient history and complete initial evaluations.
  • Consult with physicians regarding changes in treatment or condition.
  • Instruct patients and family in home programs and activities of daily living.
  • Participate in Quality Assessment and Performance Improvement activities as assigned.
  • Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and documentation of significant change in condition.
  • Utilize appropriate ICD-10 codes for documentation.

Requirements

  • Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA.
  • Active Washington State Physical Therapy license required to start the position.
  • BLS certification from AHA or ARC may be required.
  • Experience providing home health physical therapy preferred.
  • Experience with OASIS documentation and ICD-10 coding.
  • Ability to take initial patient history, perform evaluations, and develop plan of care under the direction of the Director of Clinical Services.
  • Experience directing and supervising PTAs and Home Health Aides as assigned.
  • Valid driver’s license and ability to travel within the greater Seattle area (coverage excludes west and south Seattle).
