About the role

The Physical Therapist at Hackensack Meridian Health evaluates, plans, and implements a goal-oriented Physical Therapy treatment program focusing on restoring function and preventing further disability. This role involves committee representation, customer service, delivering care, discharge planning, DME assessment, documentation compliance, effective communication, evaluation information gathering, and handling special projects.

Responsibilities

  • Participates on committees as requested by Supervisor
  • Delivers the highest level of service to all residents, families, and visitors
  • Maintains a safe environment for delivering services and employs correct body mechanics during activities
  • Completes resident treatments as scheduled
  • Establishes and updates discharge plans and completes discharge summaries in a timely manner
  • Demonstrates and implements appropriate treatment techniques and skills with residents
  • Prepares written documentation such as Progress Notes, Reassessments, and Discharge Summaries
  • Completes evaluations and gathers necessary information to properly assess the resident's needs
  • Develops and implements an individualized treatment program based on established goals
  • Ensures the proper utilization and clinical appropriateness of assistive/adaptive devices and durable medical equipment
  • Provides education to residents/families based upon the identified learning needs
  • Handles special projects and rush assignments as needed

Qualifications

  • Graduate of an APTA accredited program or equivalent
  • Standing, walking, and/or continuous movement
  • Kneeling, stooping, crouching, crawling, or working in other unusual physical positions
  • Climbing (including ladders, stepping stools, and/or stairs)
  • Pushing, pulling, lifting, and/or carrying heavy objects (20 lbs. or more) without aid of physical devices
  • Dexterity and eye-hand coordination
  • Visual ability to monitor instruments and computers, distinguish characters, recognize colors, observe physical characteristics, etc.
  • Hearing ability to recognize sounds, sound patterns, and/or other auditory stimuli
  • Physical ability to react/respond quickly in emergencies, including life-threatening situations
  • Excellent written and verbal communication skills
  • Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms
  • Physical Therapist License
  • AHA Basic Health Care Life Support HCP Certification
