Seeking licensed Physical Therapists to provide one-on-one geriatric care in residential and senior living settings.
Responsibilities
- Conduct evaluations
- Develop personalized treatment plans
- Deliver evidence-based therapy to improve mobility and independence
- Document in electronic medical records
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and staff to promote safety and aging in place
- Incorporate telehealth and virtual care to enhance service delivery
Qualifications
- Valid license
- Degree from an accredited program
- Computer proficiency
- CPR certification
Physical Demands
- Exerting moderate effort
- Assisting with patient transfers
- Traveling to patient locations
