About the Role

Assess, plan, and implement rehabilitative programs to improve patient mobility, reduce pain, and prevent deformities caused by disease or injury.

Responsibilities

• Develop and revise treatment plans in collaboration with patients and physicians.
• Provide patient and family education.
• Document evaluations and treatments.
• Facilitate discharge planning and referrals.
• Establish and evaluate treatment goals.
• Create care plans with input from patients and families.
• Ensure realistic discharge strategies.
• Record evaluations, treatments, and education according to departmental standards, adhering to regulatory and ethical guidelines.
• Participate in staff meetings, training, and ongoing education.
• Maintain confidentiality, punctuality, and a professional appearance.
• Promote a positive team environment.
• Demonstrate excellent communication and apply service recovery principles.
• Pursue continual learning to enhance patient care.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy.
• Valid Physical Therapist license.
• BLS certification.
• Effective communication and customer service skills.
• Team-oriented mindset.
• Basic computer skills.
