About the Role
We are seeking a licensed Physical Therapist for a full-time floater role in skilled nursing and long-term care settings.
Responsibilities
- Evaluating and treating patients with mobility, strength, balance, and functional impairments
- Developing and implementing personalized care plans
- Collaborating with team members to ensure quality outcomes
- Maintaining documentation to meet regulatory standards
- Obtaining and updating physician orders as required
Requirements
- Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT)
- Current state licensure as a Physical Therapist
- At least one year of SNF experience and willingness to travel among multiple facilities
