Physical Therapist Float - Full-Time (Metro Facilities)

Qualifications

• Current state licensure in Physical Therapy, in good standing
• Bachelor's degree

Responsibilities

• Develops and revises plan of care based on the residents' needs and establishes functional goals, outcomes, and discharge plans
• Plans for residents' discharge and anticipates for long term and/or short term residents' needs
• Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents
• Provides treatment to all BHS communities as needed; may be required to travel to multiple sites in a day
