How You’ll Help

Transform Healthcare
Confirms and interprets physician referrals, performs patient assessments and plans and implements individualized treatment programs taking into account the patient’s developmental and/or chronological age and medical diagnoses in accordance with the regulations governing the principles and practice of Physical Therapy.
Provides patient care based upon physician referral and age-appropriate assessment taking into consideration the patient’s developmental/chronological age and medical diagnoses when planning/implementing treatment programs designed to achieve goals set in conjunction with the patient and/or family member.
Coordinates patient care and clinical activities through written and/or verbal communication with patients, families, therapists, Clinical Team Leaders, managers, physicians and other members of the health care team.
Documents in the patient’s medical record, and completes other patient care-related forms in compliance with regulatory agencies and department/hospital policies and procedures.

What We Require

  • Education: Graduate of approved school of Physical Therapy. Bachelor's degree as Physical Therapist and 5 years of experience, OR Master's or Doctorate degree required.

  • Licensure, certification, and/or registration: Licensed to practice in the Commonwealth of Virginia by the Virginia Board of Physical Therapy.

  • AHA BLS-certified (from a Carilion approved program) within six months of employment.

  • Effective interpersonal, communication, customer service and teamwork skills required.

Impact of work is generally within department or service line.

Working Conditions

  • Work is performed primarily indoors in a clinical setting including hospital, nursing home or outpatient sites.

  • May be required to work at other Carilion facilities to meet staffing needs, in which case will provide own transportation.

  • For Home Care Departments work is performed at patient’s home.

  • On occasion work may be performed outside of a clinic setting such as work with the driving program, hydrotherapy or home evaluations.

  • Work requires physical activity with patients including lifting, manipulating, pushing, pulling, bending and walking. Patients may weigh over 200 pounds.

  • Occasional exposure to serious injury and frequent environmental hazards to include blood and bodily fluids requiring standard precautions.

  • Home care practitioners must provide own transportation and travel daily between patient homes.

Age-Specific Criteria

  • Demonstrates knowledge of the principles of life span growth and development and assesses data reflective of the patient’s status.

  • Interprets the appropriate information needed to identify each patient’s requirements relative to his or her age specific needs and provides care according to departmental policies and procedures.

Other

  • May be required to work at other Carilion facilities to meet staffing needs.
