How You’ll Help
Transform Healthcare
Confirms and interprets physician referrals, performs patient assessments and plans and implements individualized treatment programs taking into account the patient’s developmental and/or chronological age and medical diagnoses in accordance with the regulations governing the principles and practice of Physical Therapy.
Provides patient care based upon physician referral and age-appropriate assessment taking into consideration the patient’s developmental/chronological age and medical diagnoses when planning/implementing treatment programs designed to achieve goals set in conjunction with the patient and/or family member.
Coordinates patient care and clinical activities through written and/or verbal communication with patients, families, therapists, Clinical Team Leaders, managers, physicians and other members of the health care team.
Documents in the patient’s medical record, and completes other patient care-related forms in compliance with regulatory agencies and department/hospital policies and procedures.
What We Require
Education: Graduate of approved school of Physical Therapy. Bachelor's degree as Physical Therapist and 5 years of experience, OR Master's or Doctorate degree required.
Licensure, certification, and/or registration: Licensed to practice in the Commonwealth of Virginia by the Virginia Board of Physical Therapy.
AHA BLS-certified (from a Carilion approved program) within six months of employment.
Minimum Qualifications
Effective interpersonal, communication, customer service and teamwork skills required.
Position Impact
Impact of work is generally within department or service line.
Working Conditions
Work is performed primarily indoors in a clinical setting including hospital, nursing home or outpatient sites.
May be required to work at other Carilion facilities to meet staffing needs, in which case will provide own transportation.
For Home Care Departments work is performed at patient’s home.
On occasion work may be performed outside of a clinic setting such as work with the driving program, hydrotherapy or home evaluations.
Work requires physical activity with patients including lifting, manipulating, pushing, pulling, bending and walking. Patients may weigh over 200 pounds.
Occasional exposure to serious injury and frequent environmental hazards to include blood and bodily fluids requiring standard precautions.
Home care practitioners must provide own transportation and travel daily between patient homes.
Age-Specific Criteria
Demonstrates knowledge of the principles of life span growth and development and assesses data reflective of the patient’s status.
Interprets the appropriate information needed to identify each patient’s requirements relative to his or her age specific needs and provides care according to departmental policies and procedures.

May be required to work at other Carilion facilities to meet staffing needs.
