Responsibilities

• Provide education and counseling to patients and families regarding treatments and goals.
• Evaluate physical and functional status, cognitive, psychological, and perceptual abilities to determine patient capabilities.
• Document evaluations, treatments, and coordination of care contacts according to facility policies.
• Independently manage your case load to optimize the care plan.
• Comply with all relevant laws, regulations, and facility policies.
• Support special projects and other activities as directed.

Requirements

• Physical Therapist license or anticipated completion of DPT coursework and exams within 6 months.
• Commitment to top quality patient care.
• Eagerness to develop skills and support team members.
