How You Will Make An Impact
- Provide education and counseling to patients and families regarding treatments and goals.
- Evaluate physical and functional status, cognitive, psychological, and perceptual ability to determine capabilities to function safely.
- Document all evaluations, treatment, and coordination of care contacts via appropriate facility policies.
- Independently manage your case load to maximize the effectiveness of the overall care plan.
- Comply with all applicable Federal, State, and local laws, regulations, and requirements, as well as facility/agency specific policies and procedures.
- Support special projects and other designated activities as assigned.
