Responsibilities
- Performs evaluations and re-evaluations to assess patient conditions and progress.
- Establishes individualized treatment plans based on evaluation results and patient goals.
- Discharges patients by completing necessary documentation and providing follow-up recommendations.
- Documents patient care activities accurately and timely in the medical record.
- Charges for services rendered according to established billing procedures.
- Educates patients and caregivers about treatment plans, exercises, and home care instructions.
- Participates in practice changes resulting from performance improvement activities.
- Supports quality standards and initiatives set by the department.
- Maintains clinical competencies through ongoing education and training.
- Meets productivity standards defined by the department.
- Adapts to changes in patient census by adjusting work hours and assisting in other centers or programs as needed.
- Performs other duties as assigned.
Requirements
- Entry level professional competency of Physical Therapy practices, theory, principles and applications as defined in competencies departmental policy
- Basic computer skills to include Outlook, Microsoft Office and Windows
Qualifications
- Doctorate in Physical Therapy (Preferred)
- Physical Therapist (PT) license (Required)
- Physical Therapist - Temporary (PT-TEMP) license (Required)
- Basic Life Support - CPR Cert (BLS) (Required)
Education requirements for this position are determined by the licensing or certification standards set forth by the relevant certifying body. Applicants must possess the level of education mandated for eligibility to obtain and maintain the required license or certification.
