About the Role
As a Physical Therapist at EleVis Wellness Collective on the corner of W Randolph and N Deplanes in Chicago, IL, you will play a vital role in guiding patients through personalized rehabilitation programs designed to enhance mobility, restore function, and support lasting health outcomes. Our clinic emphasizes a whole-person approach, combining advanced clinical techniques with collaborative care. You will work closely with a multidisciplinary team to create and implement individualized treatment plans that prioritize patient goals and measurable results.
Responsibilities
- Perform comprehensive patient evaluations and develop individualized treatment plans
- Design and progress rehabilitation programs based on patient needs and goals
- Educate patients on movement, recovery strategies, and injury prevention
- Collaborate with a multidisciplinary team to ensure coordinated care
- Deliver patient-centered, outcomes-driven therapy services
- Maintain accurate and timely documentation of patient progress
- Assist patients with exercises, mobility training, and therapeutic activities
- Safely support or lift up to 40 lbs when necessary
Qualifications
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
- Current state licensure as a Physical Therapist in good standing
- Individual NPI
- Strong knowledge of musculoskeletal rehabilitation, biomechanics, and movement science
- Experience with manual therapy and functional rehabilitation preferred
- Willingness to learn and be trained in additional integrative modalities
Skills
- Strong clinical reasoning and patient assessment abilities
- Comprehensive musculoskeletal and functional movement evaluation skills
- Postural and biomechanical analysis expertise
- Ability to identify mobility restrictions, muscle imbalances, and movement dysfunction
- Differential diagnosis of orthopedic and neuromuscular conditions
- Excellent communication and patient engagement skills

Pay
$60/hour. Bonuses and tips are included based on performance.
