Physical Therapist Case Manager, Level 2: Home Health

About the role

The Physical Therapist Case Manager will oversee the care coordination process for patients receiving home health services. This includes managing patient care plans, ensuring compliance with regulatory standards, and collaborating with interdisciplinary teams.

Responsibilities

• Oversee the care coordination process for patients receiving home health services
• Manage patient care plans to ensure they meet clinical and regulatory standards
• Collaborate with interdisciplinary teams including physical therapists, nurses, and social workers
• Ensure timely and accurate documentation of patient care activities
• Communicate effectively with patients, families, and other healthcare providers
• Monitor patient progress and make adjustments to care plans as necessary
• Participate in quality improvement initiatives and risk management activities

Requirements

• Bachelor’s degree in Physical Therapy or related field
• Current state licensure as a Physical Therapist
• Minimum of 2 years of experience in home health or related field
• Experience in case management and care coordination
• Strong communication and interpersonal skills
• Proficiency in Microsoft Office Suite

Qualifications

• Knowledge of Medicare and Medicaid regulations
• Ability to work independently and manage multiple tasks simultaneously
• Excellent organizational and time management skills
