About the role

The Physical Therapist Case Manager will oversee the care of patients in their homes, ensuring that they receive appropriate physical therapy services. This role involves managing patient care plans, coordinating with other healthcare providers, and providing education to patients and their families.

Responsibilities

• Oversee the care of patients in their homes, ensuring they receive appropriate physical therapy services.
• Manage patient care plans, including setting goals and monitoring progress.
• Coordinate with other healthcare providers to ensure seamless care delivery.
• Provide education to patients and their families on physical therapy techniques and home exercises.
• Document all interactions and patient progress in accordance with regulatory requirements.

Requirements

• Bachelor's degree in Physical Therapy from an accredited program.
• Current California Physical Therapy license.
• At least 2 years of experience as a Physical Therapist.
• Experience in home health care settings preferred.

Qualifications
