About the role
Responsible for managing the care of patients in their homes, ensuring they receive appropriate physical therapy treatments and follow-up care.
Responsibilities
- Assess patient needs and develop individualized treatment plans
- Communicate with patients, families, and other healthcare providers to coordinate care
- Provide direct physical therapy services to patients in their home environment
- Document patient progress and communicate with supervising PTs
- Ensure compliance with regulatory standards and insurance requirements
Requirements
- Bachelor's degree in Physical Therapy from an accredited program
- Current state licensure as a Physical Therapist
- At least 1 year of relevant work experience in home health or related field
- Excellent communication and interpersonal skills
- Ability to travel to various locations within the service area
Qualifications
- Proficiency in Microsoft Office Suite
- Strong organizational and time management skills
- Valid driver’s license and reliable transportation

Pay
