About the role

As a Physical Therapist Case Manager/Kids Path - PRN, you will be responsible for managing the care of patients under the supervision of a physical therapist. You will coordinate care plans, communicate with healthcare providers, and ensure that patients receive appropriate treatment.

Responsibilities

• Manage patient care plans and coordinate with physical therapists to ensure continuity of care.
• Communicate effectively with patients, families, and other healthcare professionals to provide comprehensive care.
• Assess patient progress and make recommendations for adjustments in treatment plans.
• Ensure compliance with all regulatory standards and insurance requirements.

Requirements

• A minimum of a Bachelor's degree in Physical Therapy.
• Current licensure as a Physical Therapist in North Carolina.
• Experience in case management or related healthcare field preferred.
• Strong communication and organizational skills.

Pay

The pay range for this position is $50,000 - $70,000 annually.
