What You’ll Do Perform initial physical therapy assessments for home health patientsDevelop and revise individualized treatment plans and discharge plansCoordinate with interdisciplinary team members to create individualized care plansDeliver compassionate, skilled therapy, including preventative and rehabilitative techniques in the home settingEducate patients, families, and caregivers on care techniques and recovery to ensure comfort and understandingDocument patient progress, physician orders, therapy outcomes, and maintain accurate records in compliance with regulationsSupervise PTAs and HHAs, including conducting supervisory visitsComply with Medicare, Medicaid, and insurance regulations Drive to patient homes and work independently in the fieldProvide emotional support to patients and loved ones during challenging times Location: Southwest Atlanta You’re Perfect For The Position If You Have Doctorate from an accredited Physical Therapy programCurrent GA Physical Therapy licenseCPR certification and valid driver's license requiredReliable transportation and automobile insuranceExperience with Medicare/Medicaid regulations and compliance in home healthStrong documentation and assessment skills (e.g., OASIS, care plans, discharge planning)Ability to educate patients, families, and interdisciplinary team membersCompetency in managing rehabilitative therapy services and patient care protocols Why Work for Visiting Nurse Health System?
