Physical Therapist

About the role

We are seeking a skilled and qualified Primary Care Physical Therapist to service our patients throughout the Charleston area. This is a full-time 8-hr position (8:00AM-5:00PM) (Monday-Friday).

Responsibilities

• Performs visits in homes, ALF's, ILF's, clinics, and via Telehealth.
• Provides high quality care for our patients.
• Obtains complete health and psychosocial history and records findings in a systematic format.
• Performs complete physical assessment, while providing health education and counseling.
• Arranges referrals to other members of the health team as needed and to community resources.
• Uses appropriate reporting mechanisms as required by law, respects the dignity, confidentiality, and privacy of patients, obtains appropriate authorization and consents for treatments, reviews patient’s medical diagnosis, prognosis, procedures, and clinical course.
• Marks and provides community education.

Qualifications

• Successful completion of a Physical Therapy training program and certified thought the therapy state board.
• A current license to practice in the state of SC.
• A current CPR Certificate.
• Previous experience in healthcare setting is preferred.
• Must be able to work flexible hours and travel between offices, facilities, and other places.
• Ability to utilize electronic documentation required.
• Experience and/or interest in Pelvic Health is preferred.
