About the role

We are seeking a licensed Physical Therapist to provide home health services, delivering patient-centered care across diverse settings while demonstrating professionalism and adhering to practice standards.

Responsibilities

• Evaluate patients' functional status
• Develop and implement individualized treatment plans
• Perform interventions to improve mobility and reduce pain
• Monitor patient progress and adjust care as needed
• Collaborate with physicians and other healthcare team members
• Document patient visits and maintain accurate records
• Educate patients and families about care plans and home exercises

Requirements

• Active Physical Therapist license in the state of practice
• Valid driver’s license with access to reliable transportation
• CPR certification (preferred)
