About the role
Presbyterian Homes & Services (PHS) is a non-profit organization looking for a Physical Therapist to support our growing Outpatient needs at various PHS Independent and Assisted Living Communities. This includes locations in Waukesha, Pewaukee, Oconomowoc, Muskego, Menomonee Falls, and Germantown.

Responsibilities
- Conduct a comprehensive assessment & evaluation using subjective and objective measures.
- Develop treatment plans and programs in accordance with physician orders.
- Provide quality treatments, actively progressing goals to achieve highest quality of life.
- Conduct screens and provide education to residents/clients, families, caregivers, and staff.
- Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.
- Provide accurate and timely documentation & billing in electronic medical record.
- Demonstrate understanding and knowledge of payer sources and regulatory requirements.
- Complete and document supervisory visits per regulations.
- Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.
Qualifications
- Graduate from an accredited Physical Therapy program.
- Current licensure in the state(s) of practice as required.
- Demonstrated compatibility with PHS's mission and operating philosophies.
- Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
