About the role

The primary focus is to instruct staff members and families in treatment procedures to be continued at home, school, or at Day Habilitation. Responsibilities also include consulting with medical practitioners, service coordinators, and others to plan, implement, and assess interventions. Additionally, the role involves developing Individualized Family Service Plans, Individualized Education Programs, Individual Program Plans, or Individual Service Plans as needed.

Responsibilities

  • Instruct staff members and families in treatment procedures
  • Consult with medical practitioners, service coordinators, and others to plan, implement, and assess interventions
  • Develop Individualized Family Service Plans, Individualized Education Programs, Individual Program Plans, or Individual Service Plans as appropriate
  • Complete all documentation in compliance with applicable regulations and industry standards as needed for reimbursement
  • Prepare for the transition from Early Intervention to Preschool, to school-based services (BOCES), and then the community
  • Evaluate, fit, adjust, fix, and maintain prosthetic, orthotic, and adaptive equipment devices
  • Supervise physical therapy assistants according to New York State Physical Therapy Practice Act
  • Improve building accessibility and plan environmental modifications
  • Order and purchase equipment for agency use
  • Provide clinical educational opportunities for students of physical therapy
  • Complete incident reports as indicated
  • Conduct daily safety inspections to minimize and address safety hazards and ensure proper equipment usage
  • Complete 36 contact hours of continuing education requirements annually

Requirements

Must possess a current license to practice as a Physical Therapist in the state of New York. Proficiency with varied computer programs is required. Excellent time management, organizational, communication, and training skills are essential. Detail-oriented and able to protect and maintain confidentiality at all times.
