About the Role
Pride Health is hiring a Travel Physical Therapist to support our client’s home health services in Pensacola, FL (32504). This is a 3+ month contract assignment and a great opportunity to work with a leading healthcare organization while gaining valuable home health experience.
Job Summary
The Travel Physical Therapist will provide high-quality, in-home patient care focused on rehabilitation, mobility improvement, and restoring functional independence, while maintaining strong documentation and productivity standards.
Key Responsibilities
- Provide physical therapy services to patients in their homes
- Complete a minimum of 6 patient visits (points) per day
- Conduct patient evaluations and develop individualized treatment plans
- Deliver therapeutic exercises and rehabilitation programs
- Document patient progress using OASIS and maintain accurate records
- Educate patients and caregivers on treatment plans and recovery goals
- Travel daily to patient homes within the Pensacola to Destin coverage area
- Ensure compliance with healthcare regulations and safety standards
Required Qualifications
- Associate's Degree or higher in Physical Therapy
- Active Physical Therapist license (Florida preferred)
- OASIS proficiency required
- Home health experience preferred
- Strong clinical, documentation, and time management skills
- Ability to work independently in a field-based setting
Schedule
- Shift: Monday - Friday, 8:00 AM - 4:30 PM
- Duration: 3+ Month Contract
- Travel: Daily travel to patient homes required
