Responsibilities
- Evaluates and treats in the most functional environment which is the patient's home.
- Develops and delivers physical therapy programs to facilitate rehabilitation.
- Makes referrals to other disciplines and agencies to address the comprehensive needs of the patient.
- Selects the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient.
- Adapts program and/or plan of care according to the needs of the individual patient.
- Promotes a safe environment and enforces the elimination of fire and safety hazards.
- Orders supplies and equipment as necessary.
- Cleans, maintains, identifies, and reports the need for repair of equipment as necessary.
- Screens, examines, and evaluates patients, including history, systems review, and application of appropriate tests and measures.
- Designs, fabricates, fits, and/or applies adaptive devices, and trains patients in the use of adaptive devices.
- Selects appropriate activities suited to the individual’s physical and mental capacity, intelligence level, and interest.
- Instructs patient in the skills and techniques required for participation in mobility activities and evaluates patient progress, attitudes, and behavior as related to rehabilitative potential.
- Performs routine reexaminations as needed/required to modify/progress plan of treatment.
- Incorporates health and wellness programming into treatment plans for patients.
- Performs home and community mobility assessments and provides recommendations for safe mobility in the home and community space.
Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility;
- Must have a Master’s degree in Physical Therapy; or
- Must have a Master’s degree in a Physical Therapy related field in addition to a Bachelor’s degree in Physical Therapy; or
- Must have a Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
