About the role

At Johns Island Post Acute, we are a 132-bed skilled nursing and rehab facility located in Johns Island, SC, near historic downtown Charleston. We are seeking a licensed Physical Therapist to join our dedicated team.

Responsibilities

• Develop and implement individualized treatment plans for patients
• Perform physical therapy treatments to improve mobility and function
• Evaluate patient progress and adjust treatment plans as necessary
• Collaborate with interdisciplinary teams to ensure comprehensive care
• Document patient progress and treatment sessions accurately

Requirements

• Physical Therapy Degree from an accredited college/university
• South Carolina Licensed Physical Therapist
• CPR Certification (preferred)

Qualifications

• Genuine desire to make a difference in the lives of others
• Strong interpersonal skills and ability to work effectively with patients, families, and colleagues
• Effective communication and problem-solving skills
