Role Overview

The Physical Therapist in Home Health evaluates patients' functional status, mobility, and medical conditions to develop and implement individualized treatment plans. They provide therapy interventions to improve or maintain patient well-being, monitor progress, and communicate with healthcare team members, patients, and families about care plans and progress.

Responsibilities

  • Assess patient needs and assist in developing therapy plans of care.
  • Administer treatments within state practice laws, observing and reporting patient responses.
  • Educate patients, families, and healthcare team members regarding treatment goals and progress.
  • Complete evaluations, progress notes, and reports on patient outcomes and treatment plans.
  • Provide care in diverse home environments, ensuring inclusion regardless of background.

Qualifications & Skills

  • Current physical therapy license in the practice state.
  • Valid driver's license, vehicle insurance, or access to transportation.
  • Preferred CPR certification or ability to obtain within 90 days.
