About the role

Travel required between the Bloomer campus and the Regional Vent Center in Chippewa Falls
Schedule/Details:
- Approximately 20 hours per week
- Flexible schedule with adaptable days and hours
- Mileage reimbursement provided

Responsibilities

  • Plan, develop, organize, implement, evaluate, and direct physical therapy, as well as its programs and activities, in accordance with current rules regulations, and guidelines that govern a skilled nursing facility
  • Participate in discharge planning, development and implementation of patient care plans, patient assessments, etc. as necessary
  • Maintain treatment records, patient files, and progress notes as required
  • Participate in the development and implementation of patient assessments and care plans, including quarterly reviews
  • Collaborate and supervise PTAs

Requirements

Doctorate degree (or bachelor's / master's degree for those grandfathered in) in physical therapy from an accredited college or university
Current license as a physical therapist in the State of Wisconsin

Qualifications
