Seeking passionate Licensed Physical Therapists in Camden, NJ to provide one-on-one care to older adults in private homes and Senior Living Communities using a Geriatric House Calls™ model.

Responsibilities

• Deliver personalized evaluations, develop care plans, and implement evidence-based treatments to enhance mobility, independence, and quality of life.
• Collaborate with an interdisciplinary team, educate patients and caregivers, and incorporate telehealth for added flexibility.
• Enjoy autonomy to create your schedule and build meaningful patient relationships in a community-focused setting.

Qualifications

• Valid PT license
• Degree from an accredited program
• Proficiency in EMR
• CPR certification
• Able to meet physical demands of in-home care
