Responsibilities

• Provide 1:1 care to older adults in their communities
• Drive rewarding patient outcomes with clinically autonomous interventions
• Create and modify treatment plans based on flexible scheduling
• Work within an interdisciplinary team focused on functional outcomes

Requirements

• A valid PT license or eligibility
• A degree from an accredited physical therapy program
• Basic computer skills
• A current CPR certification
