About the role
We are seeking a Physical Therapist (PT) to provide in-home therapy services throughout an assigned territory.
Responsibilities
- Evaluate patients and establish individualized plans of care
- Deliver skilled physical therapy interventions in the home setting
- Monitor patient progress and adjust treatment plans as needed
- Educate patients and caregivers on exercises, mobility, and fall prevention
- Collaborate with physicians and interdisciplinary care teams
- Maintain timely, accurate, and compliant documentation
- Participate in case conferences and care coordination activities
Qualifications
- Current California Physical Therapy license required
- New graduates welcome to apply
- Strong assessment, treatment planning, and documentation skills
- Excellent communication and interpersonal skills
- Valid California Driver's License
