About the role

Responsibilities

  • Evaluate patients and develop individualized treatment plans based on physician orders.
  • Provide skilled physical therapy services in accordance with the physician’s plan of care and state practice guidelines.
  • Help patients improve mobility, strength, balance, and overall function.
  • Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques.
  • Recommend adaptive equipment and home modifications to promote independence and prevent injury.
  • Collaborate with physicians and the interdisciplinary care team to ensure optimal patient outcomes.
  • Accurately complete clinical documentation, progress notes, and care plan updates in a timely manner.
  • Supervise Physical Therapist Assistants (PTAs) in accordance with company policy and state regulations.
  • Encourage and motivate patients throughout their recovery journey.

Requirements

  • Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA).
  • Current Physical Therapist (PT) license in the state of practice.
  • Home Health experience is preferred but not required.
  • At least two (2) years of experience in an acute care or similar clinical setting preferred.
  • Current CPR and First Aid certification.
  • Valid driver’s license, reliable transportation, and current automobile insurance.

Qualifications

Monday–Friday schedule (8:00 AM – 4:30 PM).
