Key Responsibilities
- Aid Physical Therapists in implementing individualized treatment plans.
- Provide skilled physical therapy interventions, exercises, and patient education in the home setting.
- Maintain and document patient progress, and communicate updates to supervising PT.
- Evaluate and ensure compliance with home health regulations, physician orders, and documentation standards.
- Collaborate with clinical, administrative, and operations teams to support patient care.
- Participate in quality improvement initiatives as directed.
- Perform other clinical or administrative duties as assigned.
Position Qualifications
- Associate degree or completion of an accredited PTA program required.
- Current state licensure/certification as a PTA.
- 1–3 years clinical experience in home health, rehabilitation, or outpatient settings preferred.
- Ability to follow PT treatment plans, perform interventions, and document patient progress.
- Knowledge of home health regulations and documentation standards.
- Proficiency with EMR systems and Microsoft Office Suite.
- Excellent communication and interpersonal skills.
- Availability during business hours with occasional after-hours/weekend coverage.
