Physical Therapist Assistant - Home Care

About the role

Provides physical therapy interventions to patients under the supervision of a licensed Physical Therapist (PT). Assists in monitoring patient progress, documents care, and follows established plans of care. Works within the scope of a Physical Therapist Assistant (PTA), including co-visits with PTs and patient re-assessment every 6 visits or 30 days.

Responsibilities

• Manages a caseload, delivering services at patients' homes or facilities.
• Assists with therapeutic activities, transfers, and use of medical equipment.
• Ensures care complies with agency policies and standards.
• Utilizes technology and infection control materials during visits.
• Participates in team meetings and special projects.
• Must be able to travel and handle various environmental conditions.

Qualifications

• NY State Certification as a Physical Therapist Assistant required
• Associate’s Degree from an approved program or equivalent
• Minimum of two years clinical patient care experience
• Valid driver’s license may be required
• Bilingual skills and basic PC skills preferred
