Job Summary

We are seeking a skilled and reliable Physical Therapist Assistant to join our team.

Key Responsibilities

  • Provide skilled physical therapy treatments in patients’ homes in accordance with the established Plan of Care
  • Implement therapeutic interventions including therapeutic exercise, gait training, balance training, transfer training, and functional mobility activities
  • Observe, assess, and document patient responses, progress, and functional outcomes
  • Complete accurate and timely documentation in EMR (Homecare Homebase - HCHB preferred) in compliance with Medicare and agency standards
  • Communicate changes in patient condition, safety concerns, or lack of progress to the supervising Physical Therapist
  • Educate patients and caregivers on home exercise programs, fall prevention, and safe use of assistive devices
  • Follow all infection control, safety, and universal precaution protocols during home visits
  • Collaborate with the interdisciplinary team (RN, OT, ST, case management) to ensure coordinated care
  • Manage assigned visits efficiently to meet productivity and coverage expectations
  • Maintain compliance with state regulations, agency policies, and payer requirements

Required Qualifications

  • Graduate of an accredited Physical Therapist Assistant (PTA) program
  • Active PTA license in the State of Florida
  • Current BLS/CPR certification (Healthcare Provider)
  • Minimum 1 year of PTA experience
  • Experience with Homecare Homebase (HCHB)
  • Strong clinical, documentation, and communication skills

Preferred Experience

  • Experience with Homecare Homebase (HCHB)
  • Strong assessment and patient education skills
  • Ability to work independently and manage a flexible schedule

Pay

$40 -$41/ hr on W2

Schedule

Monday-Friday 8:00 am to 4:30 pm

Hours

36 to 40 hrs/ week

Qualifications

Graduate of an accredited Physical Therapist Assistant (PTA) program

Active PTA license in the State of Florida
