Responsibilities
- Carries out physical therapy programs for participants and assists in planning/revising program services.
- Provides education to participants, caregivers, and staff in accordance with physician orders and InnovAge policies and regulations.
- Attends to physical therapy treatment in areas such as modality, therapeutic and functional mobility, and other services outlined in the participant care plan (PCP).
- Trains participants and caregivers on personalized programs.
- Reports patient incidents or variances to the supervising Physical Therapist immediately.
- Collaborates with the Physical Therapist to determine the continuation, modification, or termination of the treatment plan.
- Educates patients and families about their care plans.
- Presents in-service programs as assigned and participates in patient care planning meetings.
- Trains Day Center aides on maximizing participant safety and functioning.
- Orders durable medical equipment (DME) according to IGCP procedures with approval from the supervising Physical Therapist.
- Performs basic maintenance of DME and department equipment, and documents repairs and purchases.
- Notifies the Physical Therapist of urgent status changes in active rehab participants within 24 hours and documents these changes in progress notes.
- Arranges DME provisions and fittings for participants.
- Ensures participants and caregivers understand the DME loan program and policies, and obtains signatures on loan agreements.
- Instructs participants and caregivers on the use and maintenance of equipment provided.
- Directs rehab aides in carrying out rehab program services and activities, including positioning, mobility, and safety, and home exercise program supervision.
- Communicates with the Physical Therapist and interdisciplinary team (IDT) to integrate services into the overall POC for participants.
Requirements
- Graduation from a CAPTE-accredited PTA associate degree program.
- Passing the National Physical Therapy Examination (NPTE) for PTAs administered by the Federation of State Boards of Physical Therapy (FSBPT).
- Valid and unrestricted Physical Therapist Assistant license in the state where services are provided.
- State-issued driver’s license, personal transportation, good driving record, and auto insurance as required by law.
- A minimum of one year experience working with the frail or elderly.
- Experience with and understanding of the state practice act as it applies to the role of Physical Therapist Assistant in a health care setting.

- Knowledge of and experience with the state practice act as it applies to the role of Physical Therapist Assistant in a health care setting.

Pay
The posted pay range for this position is $31.01 - $38.80 per hour. Salaries are determined by various factors such as qualifications, experience, and location.
