Physical Therapist Assistant
Job Overview
Comfort Creek Nursing & Rehab Center is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our growing in-house rehabilitation team. We are hiring for Full-Time, Part-Time, and PRN (Per Diem) positions with flexible scheduling options designed to support work-life balance.
Essential Duties And Responsibilities
- Provide physical therapy treatments and interventions under the direction and supervision of the licensed Physical Therapist.
- Implement individualized treatment plans designed to improve strength, balance, mobility, endurance, coordination, gait, transfers, and functional independence.
- Monitor resident progress and communicate changes in condition or treatment response to the supervising Physical Therapist.
- Accurately document treatment sessions, resident responses, progress notes, and other required documentation in accordance with Medicare, Medicaid, state, federal, and facility guidelines.
- Assist residents recovering from orthopedic, neurological, cardiopulmonary, and medically complex conditions.
- Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, safety awareness, and proper use of assistive devices.
- Collaborate with Physical Therapists, Occupational Therapists, Speech-Language Pathologists, nursing staff, physicians, and other interdisciplinary team members to coordinate resident care.
- Participate in care conferences and interdisciplinary meetings as appropriate.
- Maintain compliance with all regulatory, professional, and facility standards.
- Foster a positive, resident-centered rehabilitation environment focused on quality outcomes and clinical excellence.
Qualifications
- Graduate of an accredited Physical Therapist Assistant (PTA) program.
- Current Georgia Physical Therapist Assistant license in good standing.
- Current CPR certification or willingness to obtain upon hire.
- Strong clinical, communication, and interpersonal skills.
- Ability to work effectively within a collaborative interdisciplinary rehabilitation team.
Preferred Experience
- In skilled nursing, subacute rehabilitation, long-term care, inpatient rehabilitation, or post-acute care.
- Familiarity with Medicare documentation requirements.
- Experience treating geriatric and medically complex patient populations.
- Knowledge of evidence-based rehabilitation practices, fall prevention, and functional mobility training.
